By submitting this proposal, I confirm that:

• the information provided is accurate and complete to the best of my knowledge
• the proposal is intended to provide a clear benefit to cardiac patients in Sussex
• I have not placed an order, entered into a contract or made a financial commitment on the assumption that the Sussex Heart Charity will provide funding
• I understand that any approval by the Sussex Heart Charity may be subject to NHS or organisational approval
• I understand that a funding decision from the Sussex Heart Charity does not replace any procurement, financial, clinical, digital, estates or other governance requirements that may apply
• where ongoing costs or responsibilities arise, appropriate arrangements will need to be confirmed before funding is released
• I will notify the Sussex Heart Charity if the cost, scope or circumstances of the proposal materially change
• I understand that the Sussex Heart Charity may contact relevant NHS or organisational colleagues where reasonably necessary to assess or progress this proposal
• I understand that the Sussex Heart Charity may request further information, quotations, supporting documentation or evidence of NHS or organisational approval before making a final funding award
• I understand that an initial decision to support this proposal may constitute approval in principle and does not itself authorise the purchase or installation of equipment, commencement of a service or project, or any financial commitment
• I understand that funding decisions are made at the discretion of the Sussex Heart Charity and that meeting the eligibility criteria does not guarantee funding
• I agree to provide reasonable feedback about the use and impact of any funding awarded
