Major Project Funding Proposal - The Sussex Heart Charity

For requests over £50,000

This form is the first stage of our major project funding process. We recognise that significant healthcare projects can involve several NHS teams and approval processes, and you do not need to have a completed business case or every internal approval in place before approaching us.

If the proposal is something the Sussex Heart Charity may wish to support, we will work with you and relevant NHS or organisational colleagues to establish what further information, approvals and arrangements are required before a final funding decision is made.

the information provided is accurate and complete to the best of my knowledge
the project is intended to provide a clear benefit to cardiac patients in Sussex
I have not placed an order, entered into a contract or made a financial commitment on the assumption that the Sussex Heart Charity will provide funding
I understand that any initial decision by the Sussex Heart Charity to support this project may constitute approval in principle only
I understand that approval in principle does not authorise the purchase or installation of equipment, commencement of a service or project, or any financial commitment
I understand that support from the Sussex Heart Charity does not replace any procurement, financial, clinical, digital, estates or other NHS or organisational governance requirements that may apply
I understand that the project may require further development, due diligence, supporting information and NHS or organisational approval before a final funding commitment can be made
where ongoing costs or responsibilities arise, appropriate arrangements will need to be agreed before funding is released
I will notify the Sussex Heart Charity if the cost, scope, timescale or circumstances of the project materially change
I understand that the Sussex Heart Charity may contact relevant NHS or organisational colleagues where reasonably necessary to assess, develop or progress this project
I understand that the Sussex Heart Charity may request further information, quotations, business cases, supporting documentation or evidence of relevant approvals before making a final funding decision
I understand that all funding decisions are made at the discretion of the Sussex Heart Charity and that submission of this proposal does not guarantee funding
I agree to provide reasonable updates and feedback about the progress, use and impact of any funding awarded
